If you reside within the practice catchment area you can be considered for registration. At the time of printing the practice has a larger than average patient list and is actively managing demand for new patient registrations. Patients wishing to register should contact reception, leaving their name, address and contact details.
If your request is successful you will be registered with the practice but you will be assigned to a 'usual doctor'. Patients have the right to express a preference to receive services from a particular doctor or professional group and the practice will endeavour to comply with any reasonable preference expressed. Nevertheless, the practice has the right to refuse such a request where it has reasonable grounds to do so.
You will be notified in writing if your request for registration is unsuccessful.
ALL NEW PATIENT REGISTRATIONS ARE SUBJECT TO THE COMPLETION OF A NEW PATIENT FORM. PATIENTS ARE OFFERED A CHECK WITH A MEMBER OF THE PRACTICE NURSE TEAM.
Pre-Registration with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
